Death of the Worlds greatest 
Senna's car met the wall at a 45 degree angle, the wheel smashed into the wall and then headed toward the cockpit as the rest of the car headed into the wall. The wheel had the suspension arms still attached to it and had left the chassis.
The wheel was then compressed in a sandwich between Senna's crash helmet and the wall as the car headed toward the wall.
At this point Senna's head was punctured by the suspension arms in two places. One through the visor and one the helmet. The compression between the tyre-wall-helmet also broke his neck. He had three injuries (2 punctures and a broken neck) that all would have killed him on their own. He did however have no other injuries to any other part of his body.
He was essentially dead on impact, but Italian law stipulates that if a driver dies in a race a massive investigation is required and the race must be stopped. I have read the "gory" details in Sid Watkins book and he knew the moment he saw Senna he was dead - I won't put the medicial details here. But it wasn't pretty.
This brought in TWO regulations, both of which would have saved his life on their own.
1) Raised cockpit sides for 1996. The tyre would have been sent over the cockpit. It would not have been able to be sandwiched between a drivers helmet and a wall.
2) Teathered wheels. The suspension and tyres and teathered to the Chassis, they can not leave the car. Evident when Kimis suspension went at the European GP in 2005.
